The Centers for Medicare and Medicaid Services Office of Minority Health has published an interactive Mapping Medicare Disparities tool.  Chronic diseases, such as diabetes and cardiovascular disease, pose a significant problem in the United States, and disproportionately affect racial and ethnic minorities. The Mapping Medicare Disparities Tool aids in identifying disparities between Medicare sub-groups (e.g., racial, ethnic, and sex groups) in health outcomes, utilization, and spending. Visually displaying the geographic differences is a crucial step in understanding health disparity, informing policy decisions, and targeting populations and geographic areas for effective interventions. Data are available by state and county for the years 2012, 2013, and 2014.
