This flyer describes a project being implemented by the Massachusetts eHealth Initiative to assess the readiness of three community health centers (CHCs) – Family Health Center of Worcester, Harbor Health Services, and Lynn Community Health Center – to operationalize strategies to impact improvements in health care quality and cost control.
Key areas of opportunities have been identified:
- More efficient systems to support accurate attribution of patients to CHCs and physicians
- More complete and efficient aggregation of information about patient total medical expense and utilization
- Maximization of the implementation and adoption of existing and evolving health information technology functionality
- Increased adoption of patient-centered medical home and related care coordination
