The Measure Applications Partnership, convened by the National Quality Forum under contract with the U.S. Department of Health and Human Services, has released a report with its recommendations for selecting health care performance measures related to healthcare-acquired conditions and readmissions that would be used across public and private quality improvement and payment reform programs.
