Connecticut Health Foundation: Person-Centered Medical Homes and Health Equity

The Connecticut Health Foundation has three new publications on the implementation of person-centered medical homes and the relationship to health equity:

  • A policy brief that describes how medical practices seeking to achieve recognition as a medical home can simultaneously advance health equity
  • A chart that lists national quality standards and highlights how health equity can be advanced through implementation of practice transformation and quality improvement activities such as the adoption of electronic health records, establishment of medical homes, and creation of accountable care organizations
  • A list of references and resources, with hyperlinks to those accessible on the internet, on the issues of medical homes and health equity.
