This report funded by The Commonwealth Fund describes practical steps that medical practices can take to become patient-centered medical homes. A patient-centered medical home provides a personal physician who leads a team that takes responsibility for ongoing care for all health issues and coordinates care with other service providers. The authors from the Safety Net Medical Home Initiative have developed eight key “change concepts” to guide the transformation of medical practices into patient-centered medical homes:

- Organized, Evidence-Based Care
- Patient-Centered Interactions
- Enhanced Access
- Care Coordination
