As part of the evaluation of its Safety Net Medical Home Initiative, the Commonwealth Fund has released a self-assessment for community health centers as they seek to become recognized as medical homes. The survey tool was developed by the health services researchers and evaluators at the University of Chicago, with input from a national expert advisory committee.
A scoring process for the self-assessment is also available.
The availability of these tools may be helpful as Federally Qualified Health Centers have until August 26 (deadline extended) to apply for the FQHC Medicare Advanced Primary Care Practice demonstration projects being made available through the Center for Medicare and Medicaid Innovation, in partnership with the Health Resources and Services Administration.
