This implementation guide from the Safety Net Medical Home Initiative describes how evidence-based clinical guidelines can be implemented to improve health care quality and outcomes for not just individual patients but the entire panel of patients being served by the patient-centered medical home. Evidence-based care includes:
- Planned care/visits according to patient need
- Point-of-care reminders and decision support based on clinical guidelines
- Prioritized care management for patients with the most complex care needs
The Safety Net Medical Home Initiative is funded by The Commonwealth Fund and eight other funders, with technical assistance from Qualis Health and the MacColl Institute for Healthcare Innovation at the Group Health Research Institute. The initiative is working with five regional coordinating centers, which are each supporting the implementation of patient-centered medical homes in over a dozen safety net clinics.
