This report commissioned by the Centers for Medicare and Medicaid Services outlines the methodology for calculating pay-for-performance bonus payments for the Physician Group Practice Demonstration.  The demonstration and this methodology have become the basis for designing the proposed Medicare Shared Savings Program accountable care organizations.
