Patient-Centered Primary Care Collaborative: Evidence of Improved Quality Outcomes from Patient-Centered Medical Homes

The Patient-Centered Primary Care Collaborative has released an issue brief summarizing the evidence for improved health care quality outcomes from patient-centered medical homes.  The issue brief summarizes updated outcomes from medical homes implemented in integrated delivery systems,  private payer (health plan) initiatives, Medicaid initiatives, and other demonstration projects.
