This policy brief from the Robert Wood Johnson Foundation and Health Affairs explains the concept of the medical loss ratio, or the proportion of health care insurance premiums spent on direct medical care compared the administrative expenses (and profits).  The policy brief discusses the new requirement in the Patient Protection and Affordable Care Act that such ratios be limited to 85%.
