This is URAC’s proposed practice assessment toolkit for its patient-centered health care home. This is the first of three toolkits or assessment tools to be used to recognize patient-centered health care homes. Comments on these proposed performance measures were due August 2010. URAC is expected to finalize these measures by January 2011.
