This report from the Economic and Social Research Institute examines patient-centered care for underserved populations.  The report, funded by the W.K.Kellogg Foundation, highlights the following elements of patient-centered care:

  • Welcoming environment
  • Respect for patients’ values and expressed needs
  • Patient empowerment or activation
  • Socio-cultural competence
  • Coordination and integration of care
  • Comfort and support
  • Access and navigation skills (including medical home)
  • Community outreach
