The Joint Commission has released a self-assessment tool for health care organizations interested in being accredited as a primary care medical home.  The Joint Commission’s primary care medical home accreditation option is part of its accreditation for ambulatory health care organizations.  The self-assessment tool contains citations to the elements of performance required for the primary care medical home accreditation.
